Risk factors for atheroma: what is the strongest?

In my experience the strongest causal risk factor is tobacco smoking. But the risk factor concept is ambiguous as it defines also clinical or biological traits, symptoms or values associated with a complex disease. For instance baldness is associated with CHD. Not causally but probably through an insulinic link.
